Report of the Panel appointed by the UN Secretary-General, 22 Sept. 2003, with a mandate to examine all relevant facts about the situation before the 19 Aug. 2003 attack on UN headquarters in Baghdad, the circumstances of the attack itself and actions taken in the immediate aftermath; the Panel was also asked to identify key lessons on security arrangements and to make recommendations on measures that would assist in preventing or mitigating future incidents in Iraq or other high-risk missions.
